Answer: Fatwaa no. 034/07
1) It will be permissible to read Surah Fatihah or any other Surah of the Qur'an in Tashahhud after Durood Ebrahim as long as your intend reading it as a Du'aa and not as a Surah of the Qur'an. [Haashiyatut Tahaawi pg 154, Umdatul Fiqh pg 112] 2) It is permissible to read Surah Fatihah as part of the Du'aa after Salah 3) On completing your Du'aa read Durood Shareef and then recite "Subhaana rabbika rabbil izzati amma yasifun wa salamun alal mursalin Wal hamdulillahi rabbil aalamin" then say Ameen and wipe your face with your hands. 4) All Du'aa should begin with the praise of Allah. And there is no specific Sunnah method of beginning the Du'aa as Nabi [S.A.W] had no set and specific wording when beginning Du'aa.
